Middletown Road is a 6 train Westchester elevated station that was closed for renovations from October 5, 2013 to May 4, 2014. The station received the typical renovations of this era with green- and cream-colored windscreens and a renovated station house. The station has two side platforms for the three-track line, with the middle track not used in regular passenger service. Just south of the station (and visible for photographs via a zoom lens) is a large flyover junction with the Westchester Yard. There are fully grade-separated track connections into the yard from all 3 tracks. The Manhattan-bound track and middle express track rise-up over the yard lead tracks. Yard leads connect to the through tracks from both directions forming a wye.

Pre-renovations, the station had corrugated windscreens and a wooden canopy covering about three quarters of the platform length, with the exposed portions of the platform having just low fencing painted white. The platforms are ever so slightly offset from each other (by about half a car length) with the Manhattan-bound platform starting slightly north of the Pelham-bound one. Along one end of one of the platforms was a single 1970s lamppost with a metal, vertical Middletown sign on it.

To exit the station, two staircases lead down from about the third car from the back of a Pelham-bound train to a small station mezzanine. Inside are the turnstiles facing a token booth. These lead to two steetstairs down, one to each side of White Plains Road at a complex intersection just east of the Hutchinson River Parkway. One streetstair is to the SE corner of Westchester Avenue and Middletown Road, the other is across right near the triangle formed by the northbound service road of the Hutchinson River Parkway and a slip lane onto this service road. When the station was first built, before the building of the Hutchinson River Parkway, there were regular city streets at a complex intersection in this area named Appleton Avenue and Lang Avenue.
