NOTES: This Service change take-one has a few errors:
1. The GO on 8-9 May was canceled for whatever reason
2. Having the 3 shuttle bus stop for 148 Street/Lenox Terminal being on the M4 is also incorrect since the M4 runs down Broadway and turns and runs cross-town at 110 Street, the stop is on the M2
Also notice that on the first page: For more information pick up a take-one at your station. (This portion is normally on the notices posted in stations not on the brochure itself)

Both pages of the take-one are the same size, the pages were just scalled down differentlty.
